STEP 2: COMPLETE YOUR REGISTRATION!

Before you walk onto the field, be sure to submit these documents. After you book your training, you will get these forms via email in PDF form. 

You have two options:

1.) *RECOMMENDED WAY* | Complete the Google forms and submit them! Both the authorization form and the registration form. 

2.) Feel free to print the forms, write in all necessary info and scan back, so that you have a copy. To complete them via Apple products, open with your Preview application, insert information using the edit feature, save, and send back via email.
